Before November 2018 this sector did not exist. Cannabis sat in Schedule 1 of the Misuse of Drugs Regulations, the category reserved for substances considered to have no recognised medicinal value, and there was nothing for a clinic to prescribe.

Rescheduling changed that overnight, and what has grown since is a private healthcare market of a shape the UK has not really seen before: a legal, regulated treatment that the NHS almost never funds.

The Structural Fact That Explains Everything

Legality and availability are two different things, and the gap between them is the whole story of this sector.

Prescribing was made lawful. Funding was not. NHS prescriptions remain confined to a very small number of specific indications, which means that a patient wanting to explore this route pays for the consultation, the follow-ups and the medicine, indefinitely.

Every other feature of the sector follows from that. The clinics are private because there was no NHS route to be part of. The pricing is commercial because there is no large public purchaser. And the patient population skews toward people who can afford an ongoing monthly cost, which is not the same population as those who might clinically benefit.

Who Is Allowed to Prescribe

The restriction that shapes clinical practice is narrow and frequently misunderstood.

Only doctors on the General Medical Council’s Specialist Register can initiate a prescription for the unlicensed products that make up most of the market. GPs cannot, and neither can non-medical prescribers such as pharmacist or nurse prescribers, who are able to prescribe a great many other things.

Specialists must also work within their own field of competence, which is why a well-run clinic has different consultants covering pain, neurology and psychiatry rather than one doctor covering everything.

Registration and Oversight

Clinics operating in England are regulated healthcare providers rather than wellness businesses, and the requirements are specific.

The Care Quality Commission’s guidance on registration for services providing cannabis-based medicinal products explains that legal registration requirements apply to NHS and independent organisations alike, and that a provider adding these products to its service, or intending to prescribe them remotely through online consultation, must update its statement of purpose and notify CQC in writing within 28 days.

That last detail is a useful consumer signal. Remote prescribing is common in this sector, and a provider doing it properly has told the regulator it is doing so.

Providers elsewhere in the UK are regulated by their own national bodies rather than by CQC, which is a distinction patients outside England should check rather than assume.

Directories and How to Use Them

Comparison sites listing every cannabis clinic UK patients might consider serve a real function in a sector where providers are hard to distinguish from their own marketing.

They are best used to assemble a shortlist rather than to make a decision. The things that actually differentiate providers, the identity and specialty of the prescribing doctor, whether cases go to MDT, what follow-up looks like and what the full annual cost is, generally require a direct conversation.

The Typical Patient Pathway

The route is fairly consistent between providers.

An eligibility screening establishes whether there is a diagnosed condition and whether conventional treatments have been tried without adequate result, which is the usual threshold. A consultation with a specialist follows, often by video. Cases at better providers then go to a multidisciplinary team review rather than resting on one clinician’s judgment.

If a prescription is issued, it is dispensed by a pharmacy and delivered, with follow-up appointments scheduled to review dose and effects. Providers such as Releaf Cannabis and others operating in this space publish their own version of that pathway, and comparing several is a reasonable way to see where the differences lie.

What the Sector Does Well

Two things, and they are genuine.

Access is dramatically faster than the NHS equivalent for the conditions involved, which for a patient who has been waiting years is not a trivial advantage. And the model has produced real clinical data through patient registries, which is the main route by which UK evidence in this area is currently being generated at all.

What It Does Less Well

Three things, equally genuine.

Cost excludes a large share of the people who might benefit, and that exclusion correlates with income rather than with clinical need. Quality varies more than in most regulated healthcare, because the sector is young and the barriers to entry are lower than for, say, surgery. And the marketing across the sector is frequently more confident about the evidence than the evidence supports.

That last point deserves emphasis. National guidance in this area remains cautious for most conditions precisely because the evidence base is still developing, and any provider suggesting otherwise is not describing the position accurately.

Where This Is Heading

The sector’s future depends almost entirely on one variable, which is whether the evidence base develops enough to change national guidance and open an NHS route.

If it does, the private market shrinks toward the role private healthcare plays elsewhere: speed and choice rather than sole access. If it does not, the current two-tier situation persists, with a lawful treatment available in practice mainly to those who can pay for it.

Neither outcome is close, and patients making decisions now are doing so within the system as it currently stands. This article is general information rather than medical advice, and any treatment decision belongs with a qualified clinician.
