As previously mentioned, the playtable is integrated into the background, a unique design feature. The background of the base game has a relatively dark colour scheme as a result of the theme; however, it features plenty of brighter imagery with the gem symbols visible in the background. The animated style suits the Hot Gems Xtreme format with wooden structures in the forefront, leading to different sections of the mines and rocks in the background, continuing the mining theme. The background of the base game maintains the theme very well and has details that make it visually appealing whilst being very recognisable for the Hot Gems brand. There is an additional background in the base game that follows a theme similar to the main one. The lift lowers to a different level of the mine with a darker background, more rock design and a blue light surrounding it. Despite being a different background, it still maintains the theme of Hot Gems Xtreme well and is aesthetically pleasing.

One feature of Hot Gems Xtreme is the Waysboost, which means that every time symbols land in combination, they will be removed from the table, and new symbols will fall into their place and adding another row to the table overall, increasing the number of paylines. This can also occur with the TNT symbol in play. The TNT will explode symbols surrounding it, allowing new symbols to fall into their place. The typical Wild bonus symbol is available and can substitute for every symbol except for bonus, TNT and mystery.
